How can i improve the segmentation?

Hello everyone, I'm trying to do the segmentation of medical image (dicom format). I automatically get the value of threshold (for example in this image is 1188) (first image). However, the part that joins the femur bone to the other gives wrong (second image). I need to give separate (the black part between the two has not white pixels)

Write here, to see if anyone has any new idea for segmentation or to improve the thresholding.
