How to use confusion matrix for 3-dimensional images?

I am working on 3-D images and I want to prepare a confusion matrix for those images?

What are your two axes? What do the two axes of the confusion matrix represent? Are you measuring some number of features in your 3-D images (like intensity, shape, st.dev, texture, or whatever) and then applying a classification?
