How to generate state transition rate matrix?

I have been given with different state transition rules. It's given to me with what rate the state transition is taking place. So, I want to generate the state transition rate matrix using the rules given. I attached a pic to show the kinds of rules available to me. How can this be done through coding in MATLAB?
Can anyone please suggest

That picture is difficult to read, and is incomplete.
It appears that you have taken an image of something that you have in electronic form. Why not post the original electronic form?
