Connect Matlab 3d Array position with the simulink model

Hi, I want to connect a certain position in the 3D array with Simulink.

FOR EXAMPLE If in my 3D array 2 positions (5,5,5) and (5,5,4) have the value 1 then I want to connect these positions with my Simulink model.

How it is possible?
