Is it possible to measure the circularity property without converting image to a binary one?

I want to measure the circularity property of my image, for medical imaging purposes,but all of the commands I have found just work for a binary image.

To say how circular something is, you need the points in the circle, or at least the points along the circumference. How can you do it otherwise? I'm not sure what to check for circularity if you just have a big rectangular medical image. You first have to segment it to find the blob in it that you think is roughly circular, then measure the circularity.

I am doing lung segmentation, there are some small circles in the lung tissue, I just want to find them as well as calculating the circularity of regions in the image. But I do not know how I can measure circularity without a binary image

You can't. To say a boundary is circular (or not) you have to have a boundary. If you don't have a boundary, there is nothing to measure.
