2D Lookup Table Block in Simulink Resulting in Error: Invalid setting for parameter 'BreakpointsForDimension1'

Hi. I am trying to figure out how to use a 2D lookup table in Simulink. So I created the following arbitrary table:
Data:

Breakpoint1:

And when I try to use the Lookup Table block in Simulink, it gives me the following error:

I would appreciate it if someone could help me fix the error.

table_data = yourdata; % should be an array not a table
breakpoints1 = (1 : 5).'; % should be an array not a table
breakpoints2 = 1 : 5; % should be an array not a table

your datas are in table() form convert them to array format by doing
table_data{:, :}
breakpoints1{:, :}
breakpoints2{:, :}
in the respective dialog boxes
