Is it possible to find trajectory of the center of mass of some solid in the form of some 3-d line plot.
I have an idea to use transformations of coordinates, but I dont know how to get it from the Rigid Transform Block.
How can I get this transformations or how can I get correct trajectory without transformations?

You should connect a Transform Sensor block to the center of mass of the object you want to track. This is typically the "S" port of a Solid block. You can either connect it between two frames in SimMechanics to get relative motion, or you can hook the "B" port to the World Frame to get absolute motion.
The outputs of the Transform Sensor blocks are physical signals, which can then be converted to Simulink with PS-Simulink Converter blocks. Then, it's up to you if you want to display those positions on a Simulink scope or export the data to MATLAB.
