verify bode plot and circuit

I have been working on this circuit for 5 days now and I have not be able to figure out what is wrong with the bode plot. Can someone please build the Simulink circuit and tell what the bode plot they got? The one I got hand written on it is our hand derivation of the circuit. I know it is right. I just cannot figure out why the Simulink model is incorrect.
