Blocks Configuration for CRC calculation in Simulink

Hello Everyone, I need to do a schematic like this example in simulink: http://www.mathworks.com/help/toolbox/comm/ref/generalcrcgenerator.html

I need only the four blocks above, but I don't known Simulink. I tried many configuration with the blocks, but I got nothing because keeps giving errors and I don't known how to configure formats:

Perhaps this demo will help demonstrate the usage of that block (which looks like it is the same as this submission in case that demo doesn't ship with your version). Also, perhaps it will be more useful if you could post the exact error message that you're running into, maybe with snapshots, so contributors on the forum can help better.
