DC Motor and Ultrasonic Sensor

step 1- Motor starts to rotate in a clockwise orientation with the actioning of a SPDT switch or push of a button (such a microswitch or mini push button).
step2- Motor stops for 5 seconds as the ultrasonic sensor detects a stationary wall.
step 3- The motor rotates in an anticlockwise orientation to initiate the reversing phase.
step 4- After 10 seconds into the reversing phase, the motor fully stops.
I have to make a tinker cad model and simulate in matlab. I have tried to build the simulink matlab model but unfortunately, I am not able to attach the ultrasonic senson( i do not know where to put the sensor) .
Can someone please help me to model it properly?
I am attaching the file I created without the ultrasonic sensor
