Look up table from Excel Sheet for simulink

Hi, Can anybody tell me how to get a lookup table from excel sheet so that it can be used in simulink... Pls. Write the detailed steps Thank you

How data is formated in your excel sheet?
Generally you can read the data from excel sheet using xlsread() & create simulink model using commands new_system(), open_system(), add_block(), set_param().
