There are lots of ways to write conditional commands in Simulink. For instance, you can use Logic Operator block, If Else block, Compare To block, etc. What I suggest you is to use a MATLAB Function block since it is really easy to implement conditions as if you are writing code in MATLAB command line. Just form your logic and implement it there.
