Integrate, Load and execute a dll in a Simulink Model

Hi all

I have a package made of a C++ header (*.h), a library (*.lib) and a DLL file (*.dll).

How can I integrate teh whole package inside a Simulink model as a block (or using a S-Function) ?

If using a block, can Simulink automatically identify Input and Output ports just by looking at the *.h file ?
